Home health is designed to help you recover in the comfort of your home through skilled nurses and rehabilitation services. Home Health must be ordered by your physician when they determine that your condition requires you to stay at home for recovery and rehab. Home health is the most cost effective option for receiving care, as well as it allows for patients, and their families, to be home, where they want to be.
Calhoun Home Healthcare currently serves patients within a 50 mile radius, which includes all of Calhoun County and parts of Jackson County, Matagorda County, Refugio County, and Victoria County.
Your Doctor, RN Case manager, and rehab services team (if needed) will work together to create an individualized plan of care that will determine what services as well as the frequency and type of home health visits you will be receiving. At any time, your provider may change your plan of care, increasing or decreasing the number of visits or services provided, in order to provide you with the best quality care.
